About this article:"Duodenal feeding is a recommended strategy to decrease VAP in comparison to gastric feeding. One of the rationales for this is the potential decrease in gastroesophageal reflux that may lead to increased oropharyngeal colonization and VAP. In this randomized trial ICU patients were randomized to one or other stratregy of feeding. Interestingly, the patients fed via duodenum received more calories, had less vomiting and a decreased number of VAPs. In critically ill patients an effort should be done for administering feeding via nasoduodenal tubes "

Doripenem is a new carbapenem with a broad spectrum that includes anaerobes and Gram-negative bacteria and also active in front of Pseudomonas aeruginosa. A recent randomized trial has demonstrated its clinical efficacy in VAP when compared to imipenem. Doripenem is an effective antibiotic for the treatment of late VAP or early VAP with risk factors for multidrug resistant bacteria"
